HFE Solutions have developed in-house tools for the identification of human error which have been used by the Health and Safety Executive. We have a systematic methodology upon which to identify goals and tasks and assess the risks at each of the stages of a process. Our method captures and analyses the team structure, the individual interactions, the inter-team interactions, environmental and external influences. It considers the training, the process, the individual roles, goals and tasks.
